To register for the conference, please complete the information on the following "on-line" page:

Accomodations
To reserve a room(s) at the Phoenix Park Hotel, please follow the directions provided below.
1. Please click on this link to the Hotel Registration Page: Hotel Registration Page.
2. To select your arrival date, please click on the small blue calendar on the right side of the text box. A calendar should appear. Using the arrows at the top of the calendar, go to the month of October and click on your planned arrival date. Follow the same instructions to select your departure date.
3. After you have selected the arrival and departure dates, please select the number of adults that will be in the room(s).
4. Please verify that 14683 appears in the box for the Group Code and the Corporate/Promotion Code is left blank.
5. Once you have completed steps 1 - 4 listed above, press "Continue" on the bottom right-hand side of the screen.
6. After your press Continue, the page should refresh and you should see a listing of different hotel room rates. The first rate should be the FIVS Conference Rate of $249 per night. The $249 per night rate applies to the evenings of Wednesday, October 6, Thursday, October 7, Friday, October 8, and Saturday, October 9. Please note that if you are also reserving a room(s) for Tuesday, October 5, the room rate is $449 per night. Please select the FIVS Conference Rate and press "Continue" on the bottom right-hand side of the screen.
7. On the next page, please enter your Personal and Payment Information. Then click "Review Reservation" on the bottom right-hand side of the screen.
8. Please review your hotel registration information. If you are satisfied with your registration, please proceed to complete your registration. We encourage you to print a copy of your registration confirmation for your personal records.
Please note that we cannot guarantee rooms at the rate after September 13, 2010. We strongly encourage you to register as soon as possible.
